Flew TA-4F at Marine Corps Air Station Kaneohe Bay, Hawaii

June 25, 1969: Maj. C.B. Olsen, 37, and Maj. P.L. Elliott, 37, ejected safely before their TA-4 (BuNo 154642) crashed into Kaneohe Bay about two miles out while on approach to MCAS Kaneohe. They were picked up moments later by a SAR helicopter from the Air Station. New Castle News, Thursday, June 26, 1969.

March 14, 1979: Maj. Dean H. Matzko, 31, and Capt. David J, Dannewitz, 33, were killed ... The pilot of a Marine jet trainer (BuNo 154643) that crashed in waters off MCAS Kaneohe has been identified as Maj. Dean H. Matzko, 31, of Philadelphia. Doylestown Intelligencer, Friday, 16 March 1979, Doylestown, PA. The Navy salvage ship USS Conserver will attempt to recover a large piece of wreckage of a Marine TA-4F Skyhawk that crashed last week Wednesday after takeoff from MCAS Kaneohe with Maj. Dean H. Matzko and Capt. David J. Dannewitz aboard. Search efforts since then have turned up a flight helmet, piece of a seat and a section of tailpipe. Honolulu Star Advertiser, Tuesday, 20 March 1979. Navy divers are continuing to recover pieces of a Marine TA-4F jet that crashed into Kaneohe Bay two weeks ago and the pieces will be examined by at least one specialist from the Naval Safety Center, Monterey, CA, in an effort to determine the cause of the crash. The body of the pilot has been recovered; the co-pilot is still officially listed as missing. Honolulu Star Advertiser photo, Wednesday, 28 March 1979.  The bird was down for a tail pipe clamp and everyone was getting anxious because it was down a long time. I worked NORS (Non-Operational Ready Supply) for MAG-24 Group Supply at the time and found several. Four came in the same day. I carried one of the four to the maintenance folks who installed it immediately. The bird went up and came around for a touch and go on the flight line. As he climbed from the touch the bird split in two and crashed into the ocean, not the bay. The investigation placed the burden upon the tail pipe clamp. From Spencer Sikder.
